If you wish, you may email your address to us, and we will send you an order form and a box to ship your broken glass or bulb back to us.
For faster service, you may use your own box or mailer and our on-line order form.
Follow the easy instructions below and we'll have your precious keepsake embedded in LuciteŽ, and back to you within five weeks.

Instructions for ordering using our on-line form
- Keeping glass/bulb in it's original napkin, place entire contents in small (sandwich-type) plastic bag. We will return the napkin.
- Place bag into box with the order form, which you will print and fill out.
- If you wish to enclose a personal gift card, please enclose it with the order form and your broken glass.
- Seal box with packaging tape and send by U.S. mail, or take to UPS office for shipping to us.
($2.50 should cover the cost of postage. Check with your local post office for specific costs.)
(UPS will also accept this package. Check with U.S. Post Office if you wish to insure your package.)
In the event of loss or damage to the item, liability of The Treasured Collection shall be limited to the cost of the order. Where desired, customer is urged to obtain its own insurance coverage for greater than this amount for loss or damage. Customer hereby acknowledges receipt of this sales agreement and hereby approves the foregoing terms respecting limitations of liability of The Treasured Collection.
